STEP 1 : Download Order Form & Save it on your PC
Click on the “Download Now” icon below & save the Order Form on your computer.
STEP 2 : Open the downloaded Order Form to fill up
- Fill-in your contact details
- Enter the quantity for each design, colour and power
- State replacement period (1 month or 3 months)
- State mode of shipment
- State quantity of lens cases/leaflets required
- Save the file
*Note: To view product images, you will require an internet connection
STEP 3 : Email us your completed Order Form
STEP 4 : Make payment upon receiving your Proforma Invoice
STEP 5 : Your shipment tracking will be emailed to you
